What is Social Prescribing?
Social Prescribing is a way of connecting people with activities, groups and services in your local community that might help you feel better.
Many people who contact GP Practices have a variety of social needs which impact their health and wellbeing.

- Education & training
- Food banks
- Struggling to lose weight or stay active

There is no pill to fix these issues so we have a dedicated team that can help you connect with the right people, in the right place, at the right time.
What is a Social Prescribing Link Worker (SPLW)?

Our GP Practice, as part of One Ellesmere Port Primary Care Network, have commissioned Healthbox CIC to deliver our Social Prescribing Service. Their team of highly qualified and experienced SPLWs will take the time to help you explore your needs and what matters to you. They will be able to support and/or direct you to relevant local services using their close links with community groups and organisations within Ellesmere Port. They are also able to establish and develop their own support groups if there are gaps in our local community. This allows us to offer the best service we can for our patients.

How do I access this service?
You can contact your GP Practice (by telephone, online or in person) and ask to be referred to the Social Prescribing Service or you can self-refer by clicking below:
